Children and Youth Services
Community Services has several programs designed for the children we support to meet their needs as well as the needs of their caregivers. These include Respite Care, Children’s Health Homes/Care Management, Youth Employment Services and more.

Respite
Our Respite services offer planned short-term and time-limited opportunities for families and care providers to have some personal time.

Employment Services
ACCES-VR offers several programs to help adults and youth obtain and retain competitive employment.

Children's Health Homes/Care Management
A Health Home is a care management service model that connects all of a person’s health professionals to improve communication so that all of a patient’s needs are completely met.

Community Habilitation
Community Habilitation is a service that assists youth who either live at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them.

Community Based Day Habilitation
Community Based Day Habilitation promotes community inclusion and integration. Some of the programs offered through this service include Believe to Achieve Teens and Supplemental Group Day Habilitation.

Family Reimbursement-Family Support Services
Expenses covered under the FRP may include goods and services, respite care, community involvement, and other items.

Self-Directed Services
Self-Directed Services give people the opportunity to make important decisions about their staff and their services.

Preventive Services
Preventive Services are provided to families through a contract with the Erie County Department of Social Services (ECDSS).

Behavioral Health Services
Children’s Behavioral Home & Community Based Services (HCSB) are provided to eligible youth and their families.

In-Home Services
The Habilitation Services Department will provide home and community based skills or self-development training services to children and adults who are exhibiting deficits in independent living care skills.

Project C.O.A.C.H.
Project C.O.A.C.H. provides telephone counseling, referral, outreach and intervention/prevention to people and their families in the community.

Pre-Employment Transition Services
This program is offered to schools at no cost, with flexible scheduling to accommodate their needs. Sessions can take place at Community Services for Every1’s Jefferson Career Exploration Center or within the classroom.
