Community Services takes all complaints of discrimination seriously.  If any service recipient feels that they have been discriminated against in receipt of services or transportation, they may submit a formal complaint in writing or via phone by contacting our Corporate Compliance Officer below.

A complaint that cannot be resolved at the local level may be appealed to NYSDOT, who will review the case and contact all involved parties.  NYSDOT’s civil rights complaint procedures are detailed on the Office of Civil Rights website.

If your complaint still cannot be resolved after contact with the NYSDOT, please complete the form below and submit to the Department of Health if there are unresolved Americans with Disabilities Act (ADA) complaints within any of our programs or services, including transportation services.
