Health Homes

Our Adult Health Homes are designed to help families and individuals both in the community and in the home.

A Health Home is a care management service model that connects all of a person’s health professionals for improved communication so that all of a patient’s needs are completely met. This is done primarily through a “Care Manager” who oversees and provides access to all of the services a person needs to make sure they have everything necessary to stay healthy, out of the emergency room, and out of the hospital.

What does a Health Home do?
A Health Home offers the following core services:

      • Comprehensive care management
      • Care coordination and health promotion
      • Comprehensive transitional care
      • Individual and family support
      • Referral to community and social support services
